Health Screening Benefits Claims
CSEA members can claim Critical Illness Insurance Health Screening benefits from MetLife by completing a claim form and submitting all requested information. The Health Screening Benefit Claim Form can be found here by clicking the link for Claim Form.
Critical Illness Living Benefit Claims
1. Initial Claim: Complete all sections of the claim form for an initial claim. The illness isn’t considered reported until the claim form is received.
2. Additional Claim: For additional claims on a previously reported illness, provide supporting documentation from the healthcare provider. No claim form is needed.
3. Include Claim/Certificate Number: Include your claim number and/or certificate number on all pages submitted.
4. Provide Supporting Documentation: Submit documentation from the healthcare provider with:
- Diagnosis
- Date(s) of diagnosis
- Supporting documents such as pathology reports, surgical notes, UB-04 forms, lab results, or medical records
You can submit your claim online and upload supporting information for claims AFTER February 1, 2026. To submit a claim online, click the Access My Benefits or download the MetLife Mobile App.

Once all the required information is received, claims are generally processed within 10 business days. You can also choose how to receive your payment—by electronic funds transfer or check.
