How will I know if (and when) you received my claim information?
You will be notified by phone within three days of us receiving your claim information. During this phone call, we will review the claims process with you and advise you of any additional information needed to process your claim. We will also send a letter or email to confirm receipt of your information if we are unable to reach you by phone.
What supporting documentation for care received is required?
- Attach a copy of hospital admission, discharge, or bill for Home Care Services.
- If you are age 65 or over, please provide the Medicare Summary Notice or Home Health Plan of Treatment Statement.
- If you are less than age 65, please provide a copy of the Physicians Plan of Treatment Statement.
When will I receive my check?
When you receive your benefits check will depend on when we receive all of the necessary claim information. Checks are typically mailed within two weeks of receipt of this information, unless your claim is denied or we have to request additional information to complete the claim proofs.
